Your Initial Visit
To expedite your visit, new patient forms can be completed prior to your arrival:
NEW PATIENT FORMS
For adults, we reserve 1 1/2 hours for your initial visit. This allows time:

X-RAYS
For x-rays to be taken and reviewed when necessary

ORAL HEALTH CHECK
For the doctors to check the overall health of your teeth and gums, and perform an oral cancer screening

COMMUNICATION
To discuss any problems or concerns you may have regarding your oral health to discuss any needed dental treatment

CLEANING
For our hygienist to assess your gum health, check probing depths, and clean your teeth
To help us serve you, please provide the following information at each appointment:
- A list of your current medications
- Current dental insurance information
- Updated address and contact information
If 18 years of age or younger, the patient must be accompanied by a parent or guardian to provide consent for treatment.
Do you have current x-rays from a previous dentist? Please have these available to us at your first appointment. Most dental offices request your signature for consent prior to forwarding them. They can be mailed to us or e-mailed to us at frontdesk@eastviewfamilydental.com. Otherwise, we’ll take the necessary x-rays during your visit with us. Most insurance companies limit certain types of x-rays (such as a Panorex or a full mouth series) taken in a given time (sometimes up to 5 years!), so it may be worth your time to check.
Scheduling and Appointments:
We ask for your help when it comes to keeping your commitment to your scheduled appointments. By giving 48 hours notice for changes or cancellations, other patients who need our time and care can be appointed. We certainly understand that situations come up that make it impossible to give us notice, however, repeated late cancellations may result in a charge for a missed appointment.
We provide courtesy reminder calls, emails, and text messages 2 days prior to all appointments. If we are unable to reach you, we attempt to leave a message at the phone number of your choice.
Pre-Medication:
If you have been advised by your physician or surgeon to take an antibiotic pre-medication prior to a dental appointment due to a medical condition (such as mitral valve prolapse, joint replacement, etc.) please make sure that you have the proper medication prior to the day of your dental visit. We prefer to have your physician prescribe the type of antibiotic they want you to take, as well as the length of time it will be needed. We are happy to call refills into the pharmacy of your choice.
Safety:
Our staff takes safety seriously. Infection control is of utmost importance to our team. Standards recommended by the American Dental Association (ADA), Center for Disease Control, and Occupational, Safety and Health Administration (OSHA) are strictly enforced to maintain sterilization and cross contamination processes. These standards enable us to protect our patients and our staff.
Staff Education and Training:
With over 100 years (!) of combined experience, you can feel confident in knowing that you are getting the finest dental care available. We are absolutely committed to continuing our education as we learn and master new skills and techniques. Our staff attends classes and seminars whose levels far exceed ADA requirements.
